To register for our online services you will need to complete this form and send us a photograph of you with your photo ID next to your face. Send photo to reception.witterings@nhs.net We will then issue you a username and password.

Once you have registered for Online Services, you will be able to use the service to: order your repeat prescription, make an appointment, cancel an appointment, change your contact details, review your medications and known allergies.
